HMO – With this option, you will be able to work with providers in your network coverage and you will have access to physicals, wellness programs and other doctor visits. If you have children, they will receive immunization shots, adolescent well-care and routine checkups until they turn 19. Dentist checkups are also provided until the age 11. Preventive care is provided as well.Freedom Plan – With this, you are offered various deductible choices. You will be able to have one physical per year with no cost to you and you will be given preventive care, well-child visits, dental care and emergency room care. Liberty Plan – This option allows you to have doctor visits within your provider network group. Additional out-of-pocket costs are charged if you use a provider outside your network. Immunizations, well-child, dentistry are provided to children on the plan.
